Precedes the first trade edition; issued the same year as a limited number of specially bound presentation copies. Volume I is one of 20,165 copies; Volume II is one of 11,500. Dust jackets supplied from another copy of the trade edition (without "Kansas City Edition" lettered in red on spines); without scarce slipcase.
